This is to notify the general public that CareSource® will not be offering the CareSource Advantage® Zero Premium (HMO), CareSource Advantage® (HMO) or CareSource Advantage Plus® (HMO) plans in any counties in in the 2018 plan year. If you have one of these plans, your coverage will end on December 31, 2017.
Switch to a new Medicare plan before December 31, 2017 to make sure you’re covered on January 1, 2018, please call 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.
