NOTICE OF PETITION FOR GUARDIANSHIP

Kevin Jaceczko, is notified that a petition for guardianship of a child, named Zane Jaceczko, born to Natalie (Cody) Jones on March 9, 2006, was filed in the office of the Clerk of the Jackson Court, 111 S. Main Street, P.O. Box 318, Brownstown, Indiana, 47220.

If Kevin Jaceczko seeks to contest the guardianship of the child, Zane Jaceczko, he must file a motion to contest the guardianship in the above named court no later than thirty (30) days after the date of service of this notice.

If Kevin Jaceczko does not file a motion to contest the guardianship within thirty (30) days after service of this notice the above named court will hear and determine the petition for guardianship. The consent to guardianship of Zane Jaceczko will be irrevocably implied and Kevin Jaceczko will lose the right to contest either the guardianship or the validity of Kevin Jaceczko’s implied consent to the guardianship.

No oral statement to Kevin Jaceczko, relieves Kevin Jaceczko, of Kevin Jaceczko’s obligations under this notice.

This notice complies with IC 29-1-1-12 but does not exhaustively set forth a person’s legal obligations under the Indiana guardianship statutes.  A person being served with this notice should consult the Indiana guardianship statutes.
